When we book you in, Jane emails you a link straight to your form. That link takes you to your own secure account — you will be asked to sign in or create a password first.

Or find it in My Account

You can also sign in at our booking site and either:

  • Click the banner across the top of your My Account page, or
  • Open the Intake Forms tab.

It works on your phone

The form is designed for mobile, so you can complete it from the email on your phone — no need to find a computer.

If the form will not open or you cannot find it, call us on (613) 383-2234 before your appointment and we will resend it.
